Rural Primary Health Service

Rural Primary Health Service

The Rural Primary Health Service (RPHS) is a program funded by Country to Coast Primary Health Network (CCQPHN).

RPHS aims to improve the management of chronic conditions or chronic disease risk for individuals living in regional and remote areas of Central Queensland. To achieve this, the program provides additional allied health sessions to eligible patients at no cost and enhances access to essential healthcare services in these communities.

Objectives

To increase the number of people who are effectively managing their chronic condition (or chronic condition risk) in underserviced regional or remote communities.

To increase equitable access to sustainable primary healthcare services in underserviced regional or remote communities.

Facilitate access to person-centred, efficient and effective primary health services.

Priorities

The Rural Primary Health Service health program groups its services under the following themes.

Funding eligibility

As part of its broader support, the program provides funding for RPHS sessions in both base and outreach clinics, delivered by allied health and primary care services. These sessions aim to address the chronic condition health needs identified within each community. Appropriately qualified and trained allied health and other health professionals and/or workers must deliver the services.

What patients are eligible for RPHS funded sessions

For people with a chronic condition, a GP will need to refer them into the program by determining whether they are eligible for Chronic Disease Management services, such as a GP Management Plan or a Team Care Arrangement.

On the other hand, GPs can also refer people who don’t have a chronic condition but are at risk of developing one into the RPHS program. In this case, the service they access must correspond to the identified risk factor.
