Rural Primary Health Service

Rural Primary Health Service

The Rural Primary Health Service (RPHS) is a program funded by Country to Coast Public Health Network (CCQPHN).

It aims to improve the management of chronic conditions or chronic disease risk for individuals living in regional and remote areas of Central Queensland. This is accomplished by providing additional allied health sessions to eligible patients at no cost, and enhancing access to essential healthcare services in these communities.

Objectives

To increase the number of people who are effectively managing their chronic condition (or chronic condition risk) in underserviced regional or remote communities.

To increase equitable access to sustainable primary healthcare services in underserviced regional or remote communities.

Facilitate access to person-centred, efficient and effective primary health services.

Funding eligibility

The program provides funding for RPHS sessions in both base and outreach clinics, delivered by allied health and primary care services. These sessions are designed to address the chronic conditions health needs identified within each community.  Services must be delivered by appropriately qualified and trained allied health and other health professionals and/or workers.

What patients are eligible for RPHS funded sessions

For people with a chronic condition, a GP will need to refer into the program by determining whether a person is eligible for Chronic Disease Management services such as a GP Management Plan or a Team Care Arrangement.

People without a chronic condition but a chronic condition risk can be referred into the RPHS program by a GP. The service they access must correspond to the identified risk factor.
