25 Years of Creating Healthier Communities Together

For a quarter of a century, CheckUP has been dedicated to creating healthier communities and reducing health inequities through collaboration, excellence, innovation, integrity and compassion.

This significant milestone is a testament to the ongoing journey we have embarked on, continuously adapting to meet the ever-changing needs of our communities. We owe this accomplishment to the unwavering support of our incredible partners, members and Outreach Program providers.

Our History

Did you know CheckUP was initially named Queensland Divisions of General Practice (QDGP)? In 1998 the health system was fragmented with hospitals, GPs and primary health care services looking for ways to better work together. In this environment, the QDGP was formed and completed its first official year as an incorporated body aimed at providing support and advocacy for Queensland’s 20 Divisions of General Practice at state and national levels.

Event Cancellation and Refund Policy

CheckUP hopes that everyone who registers for the 2023 Outreach Forum and The 25th Anniversary Gala Dinner will be able to attend; however, we understand that extenuating circumstances can arise. Our Event Cancellation and Refund Policies are designed to address such situations:

The CheckUP Event Cancellation and Refund Policy applies consistently to both the 2023 Outreach Forum and The 25th Anniversary Gala Dinner.

  • Registrations cancelled on or before August 24, 2023, are eligible for a refund, subject to EventBrite fees.
  • Registrants cancelling on or after August 25, 2023, will not receive a refund. This refund and cancellation policy is not subject to waivers. However, registration fees can be transferred to another individual; the new registration invoice will be adjusted to reflect the new registrant’s membership status.
  • In cases of extenuating circumstances, CheckUP reserves the right to consider refunds after August 24, 2023. Such requests will be reviewed on a case-by-case basis.
